Very Lethargic Goldfish?! Should I Treat With Medicine Or Keep Doing Water Changes???

I don't know what's going on with my goldfish right now... my water parameters are absolutely fine yet my goldfish are not. I've been doing 25-50% water changes with aquarium salt for a few days now and he just keeps getting worse. I'm pretty sure the other one is affected now too. My fantail is very lethargic and has clamped fins, the other one has a bit more energy but is clamping his fins as well my ammonia is 0, nitrite 0, and nitrate less than 5.ppm. Could they just be major league stressed? I had to treat them for ich not too long ago and then I also had to treat my oranda for fin rot, which I think my fantail could possibly be getting now as well. I'm constantly doing water changes so it can't be the water? Right? They also have red veins in their tails... I don't know what to do and I'm scared to lose them... I have paraguard, rid ich plus, melafix, and aquarium sat. I used melafix for treating my oranda in a 15 gallon QT tank and it worked very well. And beforehand I used the rid ich to get rid of ich. Everything was good and fine when I used it but now they're not doing so swell since I've stopped meds and started using 1/2 teaspoon of salt per gallon of water when I do water changes. The tank is a 40 gallon breeder that has been up since June. The fancy goldfish are still quite new but I've had my fantail since august and oranda since September. Help is greatly appreciated, I do try and feed them peas daily if I can and use bloodworms here and there. I also have Sinking pellets and flakes. I just want my fish to be happy and healthy my fantail comes out to see me but goes back in his cave and lays on the sand without moving... the oranda does lay on the sand when I leave as well.

medicate low doses, and if you see any good difference in your goldfish medicate regular doses

I just recently got him from petsmart a few months ago so he shouldn’t be too old. He’s usually very active, in one night he can easily sift through all the sand in the tank. It’s funny cause I can see all the indents from it.

Well good news is that he’s doing much better! I’m using some paraguard and they seem to both have perked up a lot over the weekend. I’m mostly watching my fantails fins because they seem to still have some bits of white on them -.- he had a tear that had a white edge to it on one of his smaller fins and it has gone away with meds and water changes. So I’m gonna do it for a bit longer until it’s all gone and my tank should be free of diseases or parasites. Haven’t seen him lay down motionless in his cave so that's great! He’s back to sleeping while floating above the sand in the corner again. they’re both back to normal again. He’s always hungry so that’s good, when I walk into my room they always come up to me instantly. I give them lots of peas and bloodworms as a small treat. He’s not scared to eat from my hand anymore. I’m just relieved they are both doing better. I did a larger water change yesterday and removed like 12 gallons today since I could see a lot of their poop. I heard about your guys fin rot? Is he doing better at all? I used melafix when my oranda got it and it worked very well but he started flipping out due to the meds, and the water quality was fine in the QT tank. So I used paraguard this time and water changes and everything is going very well!
