The last few days she has been displaying this behavior. Often she hides behind rocks and under plants. None of the other fish are behaving this way. The tank is getting weekly 25% water changes. Food hasn't changed. No unusual algae or other factors. There is another female ram in the tank that appears to be just fine. The way she stay at the bottom and breathes heavy really has me worried. Any help would be appreciated!

That could be it. If only one fish is exhibiting the symptoms should I pull it out and treat it severely or treat the whole tank?

No it must be introduced I believe ? It is a parasite so although poor water does not help it will not cause them.
Carbon is highly over rated and if not being used for filtering your tap water is useless or at least will do nothing water changes don't do better for cheaper .
Good clean warm water will help.Always do a 50% water change before any med to remove organic waste in the water and allow the med to perform to its best ability.
