Persistent Fin Rot. Need advice.

Cross posting with r/bettafish because I am desperate.

My fish has really bad fin rot which has not gone away despite several different treatments and I could really use your advice. I am all out of ideas.

Symptoms:

Lethargy, badly frayed fins which have spread to his pectoral fins, heavy breathing. He is also very skittish and seems fearful of me, although that has improved since he was moved a breeder box. His poop is stringy but it doesn't wiggle so I don't think it is parasites.

No loss of appetite, though. He will still eat himself to death if I allow it.

Tank conditions:

The tank is very mature and cycled. Ammonia 0 Nitrite 0 Nitrate 20 PH 7.5. Water temp is 77 degrees year-round. He is housed in a heavily planted 10g tank with about 50 neos, 1 zebra nerite, 2 tiny horned nerites, and 6 pygmy cories. I do a 30% water change every 1-2 weeks. The filter is rated for 20g so filtration capacity is great. No issues with water quality and the other inhabitants are health and active, except for the occasional failed shrimp molt.

Treatments:

  1. Tried IAL tea and stress guard for a couple of weeks. Didn't work.
  2. Tried 1.5 tbs of aquarium salt per gallon for 10 days in a hospital tank. Didn't work.
  3. Tried Maracyn 1 and ICH-X at the dosage recommended by Aquarium Co-Op. 1 packet and of Maracyn + 1 tsp of ICH-X per 10 gallons of water. No additional dosages. Monitored for 1 week. Didn't work.
  4. Tried Maracyn 1, Maracyn 2, and ICH-X at the dosage recommended by each manufacturer for almost 6 days. This involved adding more medicine daily. Didn't work.
  5. He is currently in a breeder box that shares the tank water with my main tank. This way I can keep an eye on him and feed him easier (otherwise the shrimp and cories will steal his food). He has a big IAL in there with him so he has a place to hide and rest (there is some debris visible in the water from this). Fin rot is still there and progressing slightly. No signs of new growth obviously. I am preparing for the worst and he is being spoiled with a 100% diet of his favorite food, which is frozen brine shirmp.
History:

I adopted Tritan from another redditer who was fostering him because his original owner neglected him and ended up moving out of town. He came to me very weak and sick, with a lot of fin rot, but I was able to rehab him with some stress guard and IAL tea. That was approximately 1 year ago. I am guessing he is about 1.5 years old.

I'm no expert but it's probably a bacterial infection over fungal.
If you can, get him in his own established water, it could be something from your other tank.
I wish I could help more. Poor guy...

Thank you. I might try a double treatment of another antibiotic and paraguard before I give up. Will be pretty bummed if I cannot save him.

My best advice is, If you can, set up a separate Quarantine tank.
Use media from other tank to jumpstart it.
This way you can eliminate other causes from the main tank. Look into what chemicals (like Bettafix) are best for finrot. Since natural methods haven't seemed to work. It can be used with aquarium salts as well I believe, but please fact check that.

The "fixes" even the Betta one are not good for bettas. It is essential oils in water and the bettas labyrinth organ doesnt do well with those "meds". From my understanding they work as a astringent for a scrape or something and thats about it. Im really not sure how they can market those for bettas. Its sad.

I would up your water changes a lot. Fin rot will only heal if you have super clean water. If you want to try something I would get some stress coat dechlor. The IAL will help also. I think you may want to up your water changes to 50% weekly. Maybe a few 20% in between. Especially if you are trying to heal fin rot. Clean water is going to be your best bet right now. :)

Thank you StarGirl and AloeInkblots. I will lay off the meds and just try 30% water changes daily in the main tank. I don't think the shrimps will tolerate big 50% changes well so I will put in the work throughout the week to hopefully achieve the same effect more gradually. The tank also has a UV filter that I forgot to mention.
