Question Leopard Danio with Strange Growth on Head [PICTURES]

I recently moved my Danio school (and Oto cat school) to a larger 29-Gallon Tank. The move went well, they are all doing alright so far. Water parameters seem to also be alright (7.4-7.6 PH, 0 Ammonia, 0 Nitrites, 5-10 Nitrates) -- I reused some of their gravel from the old tank.

However, even before I moved them, I've been noticing one of my danios has developed a strange growth on the tip of his head. His fins also seem to be ailing. I believe he's been like this for over a month. At the same time, however, he can still swim normally and still eats normally. And none of the other fish have developed similar conditions as far as I can see.

In general, I feel like I'm doing something wrong in this tank (it's still WIP). I'm trying to get to the bottom of it -- unsure if it's several little things or one more major thing that I'm missing. Having some algae growth and things -- I'll be asking about this in another post. Water parameters seem fine, though. Keeping up with weekly water changes, too (60%).
