Help Angelfish Sick No Idea What It Is!!!!!

Hello I am new to fishlore, recently I noticed my 5 and 1/2 year angelfish hasn't been feeling well it started with a little bit of tearing of the fins. Then I noticed my angel started to get cloudy eye. So I did water changes cloudy eye got better. But in the last week the Cloudy eye has returned my female angel fishes fins side and top and back fin or deteriorating at a rapid rate also it looks like her scales are coming off. She looks slightly bloated and she's hanging at the bottom of the tank with her head pointed down kind of breathing heavy I recently did a massive water change to try to help with the issue. After I did the water change the fish decline in health. She stopped eating and now she just hangs at the bottom of the tank barely moving. I have a 120 gallon tall hexagon tank with a Sump System I have four angels, 4 discus, I have one Golden Nugget pleco and two rubber nose plecos and three small Tetris. And one small albino Cory cat. My pH in my tank is around 6.8 my ammonia is a round .25 PPM, my nitrite are 0 and my nitrates are between 5ppm and 10ppm. And I keep my tank around 80 degrees. I've had this angelfish and see was the size of a nickel I really do not want to lose her if you have any ideas that can help me figure out how to treat her properly I greatly appreciate it. Also all my other fish in my tank are perfectly healthy it's just that one angel. thank you for your help

Hello. Welcome to fishlore.
I’m sorry your angel isn’t feeling well. Can you tell me what the fins look like? As in how much of each fin is affected? How many rays are bare. Any discoloration? I’ll show these pictures to my hubby when he wakes up.
Can you put her in qt? It’ll be easier to treat her and also easier for her to eat. Is she pooing? Can you get a picture from directly above her? To show her belly?
I’ll ask for eyes as well.

Her fins are clamped where there shredded I can see the barbs she still has them they just look like they're growing shorter and shorter. I really can't tell if it has discoloration she's mostly black with just a few patches of silver. She's already in her own 10-gallon quarantine tank. I was treating her with with Metro and she was pooping White so I thought she had parasites. But as soon as I did my water change and started the treatment she took a turn for the worse she is now in the quarantine tank at the top of the tank swimming on her side. Breathing heavy. I am very worried

She got worse after one treatment of metro?

Before I did my water change and treatment of metro, she was swimming around and eating fine, she still had shredded fin and her scales were looking bad but she still was eating and acting normal.I have giving her 3 treatment of metro so far. 1st she stopped eating then she was on the bottom of the tank with her head down and twitching a little and by the time I got home 9 hours later she was on the top of the tank on her side she still moves around like she's trying to swim back down but she stays at the top

HI Gypsy. The fins are tattered, don't look too bad, just the upper 1/4-1/3, but every fin has some kind of damage. It doesn't seem as jagged or tattered as finrot in some places, seems like chunks taken out between the rays in some places, maybe fin melt or nipping that led to finrot? The more concerning part is pink/red splotches where I assume scales are missing, all over the body, they seem to be random patches, though most are verticle lines and ovals, seem to be more angular. One splotch is a horizontal oval about twice the side of the eye, all the rest are thinner than the eye, most are just lines

Greetings and welcome to Fishlore

At over 5 years that is an old angel. The longest I have had one live was 7 years but generally they live 5-6 years.

Thank you all for your help, but my angelfish die this morning and I know it wasn't from age because her mate is the same age as she was and is doing great.
