Goldfish Losing Scales!! What should I do?

Information:
Ammonia: 0ppm
Nitrate: 20ppm
Nitrite: 0ppm
pH: 7-7.5
KH: 40ppm
GH: 60ppm
Fish: 1 common goldfish (~2 inches)
Tank Size: 60 Gallons
Water Change: 30% Weekly, last water change was on Wednesday (Aug 19)
Changes to Tank: Added new rocks and airstones (about 4)

Recently I noticed my goldfish has been losing his scales. At first it was a few scales a few days ago but today he has lost a lot more. I'm not sure if the cause is the items in the tank or a disease. On Wednesday when we did his regular 30% water change I also added Melafix to the tank just in case. I noticed that he is also rubbing his body against the glass from time to time. What should I do? Should I continue adding Melafix? Should I remove some items from the tank? Please help!

I also want to ask how I can get the KH to be higher? Everytime I do a water change the Ammonia, Nitrate, and Nitrite levels seem to be fine. The pH seems to stay around the 7.0-8.0 level as well but the KH seems to be a little too low all the time. I read that water changes help but I do the water changes weekly so could it be something else?
