Goldfish gasping and red spots

i have an oranda who i’ve just noticed is gasping for air at the surface, but he doesn’t usually do this. he has two red spots in his wen, which he’s had in the past but it went away with water changes and i think melafix. well it’s been a few months but they’re back and they’re just in his wen, it could possibly be ich, or an ulcer, the red spots have been there this time around for maybe a week. i took about 20 gallons of water out and replaced it, it’s a 55 gallon tank. i also put melafix in the water to help prevent a bacterial infection. his only other tank mate is a black moor, who is not gasping for air, but looking for food instead. these photos are from when i first noticed them. any ideas on what it is and how to treat it?

thanks :) i use test strips, they’re showing 0 nitrites, oh wow the nitrates are still up since that water change, they’re reading 30. it’s time for another water change. the ammonia is 0. thank you for your response, i look forward to your input!

Hmm honestly I dont know what’s going on he could just be hungry? Mine gasps for air when he’s hungry which is a lot. As for the red spots do you have any decor that could of potientally cut him?

nope just live plants, jungle val. i read that live plants can take up oxygen in the water at night so maybe that’s why he was gasping, because he isn’t doing it today. i’ll keep an eye on him tonight to see if it happens again. i also read that spots in the wen could be a bacterial infection?
