Found betta dead with hole in stomach

Hello everyone. I am completely new to this forum. I have encountered quite a disturbing matter that I'm hoping someone could maybe provide some insight on.

This morning, I woke up to find one of my bettas dead, and laying with its face towards the gravel. The very disturbing part is that upon further inspection, I could see she had a hole in her stomach. It had seemed a little unwell a few days ago, and was not eating. A day later, I put food in and it was eating again as normal. But I did notice what seemed to be a bit of fin rot as well as its stomach seeming to look swollen and pale, and whitish. I do 25-30 percent water changes every 2 weeks. My aquarium has been cycled and established for several months. All water parameters came back fine as of a week ago ( checked with api masterkit ) . What can cause this to happen? I don't think she was constipated. And she was able to swim just fine. Okay one day, gone the next. I have attached photos to show the condition she was found in. Quite horrifying.

The issue was possibly a bacterial ulcer. The last & usually fatal stage of an intestinal bacteria infection.
