I use them, and they seem to work. They're nice and porous like pumice stone.

Nope, you don't want to change them out at all. This is what holds you BB. You do want to rinse them off in some tank water when your cleaning your filter, and that's about it.

Well I just bought of Fluval Biomax, and put a couple of pellets in to all my filters (all HOB).

Can I leave the biomax in the aquarium or do I have to put them on the outside the aquarium on a HOB filter?

I've got them in my Fluval 406 canister. As stated they keep your bacteria going which helps keep your tank in good shape.
