Do I need to treat the whole tank?

No treat just the fish. If it spreads to other fish, quarantine them too

That actually looks a lot like cottonmouth disease, a bacterial infection. How long have you had this fish? When was the most recent addition?
Cottonmouth is also called columnaris and is a bacterial infection which resembles a fungus. It is extremely fast moving and can wipe out a tank in mere days, sometimes just 48 hours. You'll need strong meds to treat, try a mix of Furan 2 and Kanaplex.
Can you please post some more pictures so I know I'm not jumping to conclusions?

I have had the fish for almost two moths. I did add some neon tetras with one random death and some corys the last weekend but no deaths from them. Do I treat the whole tank? I have a rhino pleco in there too. I live far from my local fish store but a petsmart is nearby what med can I buy from there?

The neon tetras could have brought it in, especially because you have an unexplained death. With this illness, not all fish show symptoms.
As for PetSmart meds, I don't think they have anything strong enough. Any chance you can order some medications off Amazon?
coralbandit hope you don't mine me calling you in, but you seem very informed on this. What do you think?

He died in a quartine tank while I was away should I still treat the whole tanks if none of the other fish are showing symptoms?

Wait before you treat the whole tank as it can alter beneficial bacteria, killing it. Carbon absorbs the meds and can throw the whole tank stability.

Remove the carbon from the filter and treat the tank. If you want, you can try and save the carbon cartridge in tank water, but there isn't much use since half the beneficial bacteria will die every day without an ammonia source. Columnaris moves extremely quickly, and since more than half the fish afflicted won't show any signs, I would treat your whole tank.

Furan 2 and Kanaplex should both be safe for them.
