Fin rot in fancy goldfish?

A few weeks ago, I noticed that my goldfish was starting to get these little tears on his fins. Upon discovering this, I moved him to a hospital tank and followed the instructions in this video (
) to treat for fin rot using Ich-X and Maracyn. After about a week in the medicated hospital tank, I moved him back to the main tank. He has now been back in the main tank for about two weeks, but it does not seem like his fins are getting any better. A couple days ago, I also added one dose of API aquarium salt to the main tank according to the instructions on the box, but I’m not sure how much that will help.
Is there anything else can I do to help this poor guy?

My water parameters are:
Ph – 8.0
Am – 0 ppm
Ni – 0 ppm
Na – ~ 5-10 ppm
Kh – ~ 89.5-107.4 ppm
Gh – ~ 143.2-161.1 ppm
Phos – 2.0 ppm

Do you have any other fish in the main tank? Asking more so if you have noticed any similar symptoms on any other fish or just this one. Does your fish flash or rub/bounce off of anything in the tank?

Depending on things not sure whether to advise to treat entire tank or move him back to hospital tank.

If you could fill out the emergency template that would really help to be able to give you best recommendations for you and your fish’s situation.

There are 3 other fish in the main tank: 2 telescope goldfish and 1 lionchu goldfish. The one with the fin tears is a pearlscale goldfish.
None of the others seem to have any tears in their fins as far as I can tell.
Other than the fin tears, he seems to be acting normal - no flashing, no rubbing on things in the tank, not lethargic, and eagerly takes food.

How has its behavior and appearance changed, if at all? No behavioral changes, just tears in the fins

Your tank looks lovely! Sorry this is happening with your fish.
Unless your are considering salting the main tank more, you might want to put back in hospital tank/bin/etc. I’d do a 0.10 salt solution to start, so if 10 gallon 1/2 cup, 20 gallons 1 cup. Or 2.5 cups for the 55 gallon minus what you’ve added in. It sounds like a lot but goldfish even fancies have a high tolerance, this is on the low end. Maybe couple that with medicated food if red streaks in tail start to develop (though hopefully not!)
You already tried ichx and maracyn just recently so I wouldn’t throw more meds at fish just yet unless absolutely necessary.
It’s hard with goldfish sometimes because they can not be well but are active and hungry as usual.
If you don’t want to put back in hospital and are confident with doing dips (easier with fancies imo) then salt dip. Something to stop the fins from receding further.

Maybe a salt dip every 3 to 5 days.
This one is tricky because there are no other outward signs of distress.
Oh and would leave in hospital tank until fins start to show marked improvement if you decide to go hospital tank route. So could be a couple of weeks.

not getting better might not be a bad thing, it does not appear to be fin rot, just some tearing, healing fins usually take weeks or months.

as long as its not getting worse, just keep up with the weekly water changes.

clean water prevents many illnesses and promotes healing.

That’s true. Also is it possible that this fish is getting bullied by any of the others? So not a sign of infection at all but fin nipping?
