Facial Deformity In Fish?

So one my my great friends was ready to bring a new Betta fish home, he lived in a elated 5gallon filtered tank with established media and water levels all seemed normal. She told me when she got the fish he looked neglected, but I’m clean water— very skinny. After good care there was no improvement but once I saw him I was instantly confused. This guy to me looks deformed but I need help. I’ve researched and we’ve treated I case there was an infection in the mouth without much luck. He can’t eat pellet food, only flakes. If you look you will see how his head looks very misshapen compared to a normal betta with larger lips. Is this an illness or a deformity? He has not put on weight and is very skinny but also doesn’t show the signs of interstitial worms.
