15 Gallon Tank Does my angel have ich or cottonmouth disease?

I recently noticed my angel has little white spots near his mouth and also his mouth seems a bit weird to me but I cannot tell if it looks fuzzy or just normal. He is showing no symptoms of illness, eats like crazy, super active. The other angel does tend to peck on him quite often but he doesn't really seem too bothered by it.
All parameters are perfect, I have a fluval u2 filter and a heater (currently on 28.7°c), there is another angel living with him and 2 nerite snails. I change the water every 7 days or less if needed.

 

I'd say just his coloration.

His spots look kind of flat, and the examples you showed seem to have bumps.

How big is the tank that has a Fluval U 2 filter? Because it’s rated for only 15 US gallons/ 55 litres . Unless you have other filters in a bigger tank the tank size may likely be a large part of the issue leading to stress and hole in the head. You say it is being picked on , that could also be tank size related?

15 gallons is way too small for angelfish. They need 50g+ size tanks.
