?callamanus protrufing from guppy gonopodium

Hello guys!
I have a very puzzling issue with 1 of my male guppies.
It literally looks as though he has a callamanus parasite protruding from his gonopodium!?! But I mean that’s not possible is it,the gonopodium isn’t lined with the intestines etc.
He has ballooned up since around a week ago,i tried cooked shelled peas,he loved them but he still seems to be getting bigger as though he has dropsy but without the pinecone effect,so I treated with esha 2000 for the standard 3day course but it hadn’t helped, also he’s eating great but not so much pooping, then a couple days ago I noticed a callamanus looking worm hanging from his back end,my LFS advised it sounded like callamanus and advised to treat the entire tank (I have just male guppies and 2 albino corydoras in that tank) with Esha NDX as I already had some,so I did this and sat waiting to siphon up and callamanus that were passed, they were due to be fed so I fed them hoping it would help him pass the parasites, but what happened next shocked me!
The male then began pooping clear stringy poop followed by fish food coloured poop, this highlighted that the callamanus looking thing wasn’t in fact protruding from his anus but was actually protruding from his gonopodium!

I hadn’t actually connected this but over the last few weeks I have had a few guppies (although it’s been females only and my females are in a completely separate tank and everything gets cleaned between use) die, they seemed to have dropsy but some didn’t get the pinecone effect either, esha 2000 didn’t help and they died, but the weird thing is it looked as though they’d literally burst a hole out their abdomen, not right through them or anything just to the pinkish flesh, all parameters were good so I put it down to dropsy, but now I’m wondering if it’s connected,I mean I can’t see how because I clean all equipment after use so nothing gets put into 1 tank then straight into another, but these things have ways of happening no matter how careful you are,but thought I’d mention it just in case it is relevant.

I’ve attached some photos of him to help.
The first 2 were before he began pooping in colour (I feed tetra prima mini granules so my fish have bright orange poop lol) and the third is with him pooping in colour and clear string so you can see the callamanus looking thing isn’t actually protruding from his anus.

it’s absolutely confusing me so I’d be grateful for any advice you can kindly give me please guys in hopes that I can save this gorgeous little fella.

As I’d already administered the NDX treatment,he’s due for a 50% WC tomorrow and a retreat in 2wks,I think it’s probably best to complete the course of NDX now that it’s been started,I don’t think it would harm my fish if they don’t have callamanus as I know a lot of people treat quarantined fish for bacterial, fungal, parasitic and nematode treatments as a precaution, but the issue I have now is that it’s now in question as to what this red thing is on his gonopodium, what’s causing the ballooning and what do I need to do to try to save him,he’s a beautiful colour and metallic overlay and I really don’t want to lose him if possible.
Thank you ever so much guys and I wish you all a wonderful weekend!
Stay safe and god bless.

GuppyOverlord11 Thank you so so much for replying,I really do appreciate it.
He seemed famished when I fed him today like he’s never eaten before,he’s normally a good eater but today he was frantic for the food,he is swimming but he’s struggling (possibly because of his size and/or pressure on his swim bladder) and he does have a slight wobble when swimming, but apart from that and the bloating and that weird red thing he seems ok in himself, but we all know once a fish is poorly that can go downhill very quickly so I am really worried about him but I feel that right now he is trying to fight and survive as opposed to just giving up eating and succumbing to fate kinda deal.
I’ve discovered another male that was hiding behind the driftwood just laying on the floor for hours,he looks really thin, wobbles when swimming (similarly to the bloated male but this little guy is like REALLY slim) and he can’t seem to slow-swim unless his nose is pointed up and his tails pointing down but prefers to just to lay on the substrate, his colours look off too, kinda like there’s something covering him,it’s hard to explain but I’ll get a photo in the morning (it’s 2:40am here atm but I will get a photo of him tomorrow,if there’s anything else I can do to help give you a clearer picture of what could be going on with my fish please let me know overlord).
I have a Indian almond leaf in each of my tanks to help with natural antibacterial and antifungal properties and in their tank I’ve arched it over to make a little hideout for the guppies and this guy hides inside that most of the time, unless I roll up to the tank to say hello and hang out with them or it’s feed time but even though he joins in he doesn’t seem to be eating whereas the bloated male seems starved,so even though there’s some similarities I’m not sure if it could be the same issue(s) affecting both of these males.

I read up on esha NDX/levamisole (sadly we can’t get levamisole in the uk so Esha NDX is the closest we have) and it said callamanus should start to expel within 4hrs after dosage, but that red thing doesn’t appear to have even moved let alone been expelled and it’s been over 15hrs,but then I guess that’s meant for when the callamanus are protruding from the fish’ anus.

Thanks again GuppyOverlord11,I really do appreciate your reply and trying to help me to save my fish,it really means a lot, thank you so so much :)

Thank you so much for your honest opinion Overlord,it’s tough to hear but I really appreciate your honesty,it’s such a shame as he’s a beautiful little guy.
I’m also worried that whatever is wrong with him could be passed to my other fish, especially with the other male the way he is :(

You will need to deworm your tank 100%. I also recommend putting the fish down :(
