Betta fish registry- Just for fun:)

Hey! I was thinking around today, and thought about how fun it would be to make a betta fish registry. If you don't know what an animal registry is, it's basically just a bunch of registered animals of a certain type. while the are organizations for betta fish, there isn't really an actual registry. So I thought I would make one for the betta Splendens species.
So basically, all you have to do is fill out a form, and give your betta's information to me.
This will be the form below. I might make it more complicated in the future, but I'll leave it somewhat simple for now.
But, for the people who want to take an extra step, you could make up a special "School name" for your bettas. This would go right in front of the betta's given name. Totally don't need to, but it adds a bit of fun Like mine is Storm Haven, so my fish Apollo's full name would be Storm Haven Apollo.

I think that he would be called a galaxy koi due to the blue colorations; it's hard to pin point these days as many mixtures don't always fit a pattern, and then are just labeled as "fancy". I have read though that true koi shouldn't really have any blue. Regardless, he is stunning!!!

Thanks. I knew he was some sort of koi. I bought him from philam on here so I could ask lol!

Based on what I have seen for Koi Bettas, he looks like he could be either be a Nemo Galaxy Koi, or a Nemo Multicolor Koi. Either way, gorgeous fish!
