Betta bacterial infection or something else?

Recently switched from a 5 gallon that he was in for over a year to a new, fully fishless cycled 10gal. Heater and filter running, temp is 82. Live plants and some wood from the LFS. Lives with a few shrimp and some pest snails.

Noticed this facial swelling today. He also seems to be more lethargic and swimming lobsided. Checked the water parameters and everything seems good. pH between 6.6 and 6.8, ammonia 0, nitrites 0, nitrates 10. Does this look like a bacterial infection or something else? Any suggestions to treat it?

Yikes! I haven't seen anything like that before! Could he have run into something? Could you fill out the Emergency Form so the forum has a little more information? I'm not well-versed in bacterial infections but with more information, I could make a better guess. Thanks! Hope your betta's okay.
