If your aquarium info is correct and you have a 3.0 ammonia reading on that tank, the fish is likely dying from ammonia poisoning.
If you haven't cycled the tank as Amanda suggested, you need to get your water chemistry under control immediately if you want any of the fish to survive. Read about
the nitrogen cycle, and begin doing daily 25-50% water changes until you see no ammonia, no nitrites, and less than 20-40ppm nitrates. Make sure when you add the new water that you use a dechlorinator, such as SeaChem's Prime. Prime also helps detoxify the ammonia that is present, which might help the fish survive.
