I have had two Red Belly Piranha in a 50gal tank for 4 years now. A few months ago one of them looked like the other had chewed some of the tissue around the other's face. So, I treated the water with some Melafix in an effort to promote healing.
Upon further examination I can see that there is a distinct pattern around the eyes of the piranha, bio-symmetrical, almost like a rorschach of missing flesh. I have changed and tested the water several times and the other piranha shows no signs/symptoms.

(I realize they probably would do better in a bigger tank with several more piranha and would be willing to donate them to whomever had the perfect set up, but since they were gifted to me four years ago... we've done the best we could with them. We even looked into donating them to the zoo, but came up short)

I'm sorry your fish is worrying you. Four years is a good long time for fish.
I don't know about Piranha but Have you looked into Hole In The Head disease?
Can you post a pic? It might help members ID what's going on.
