Have no idea what it is...

Hi
As usual,I'm posting my question here as I know I will get good advises here. Thanks everybody

Back to the problem, last night,I did 20% water changes, vacuum the gravel and scrub the glass.
After I finished,I noticed one of my neon tetra is acting weird. It sits at the bottom, the colour faded and not eating.

I moved it to 4 gallon quarantine tank and haven't decided what to do and which meds should I use. What should I do now?(To make things worst,I'm going on vacation in 2 days from now,)

Here is the video of my sick neon tetra.

You might notice that the gills cover are slightly injured. This unlucky neon tetra had it head stuck in the filter last month and had fully recovered.

ammonia:0
nitrate:0
pH:7.6
Temp:26
Sponge filter
Feeding with Tetra Min Flake(1-2 times daily) and frozen tubifex (once daily).

Awww, poor little guy. Is his eye bulgy or it is just the way the video looks?
It looks like he's got some gill damage.
Do you have a nitrite readings? I'm curious because you have a 0 nitrates.

poor guy....i agree with Lucy his gills look pretty bad. normally thats a sign of ammonia poisoning or lack of oxygen. i don't think 26C is high enough to cause that kind of gill damage, but you may want to add an air stone anyways. what kind of test kit are you using? do any of your other fish look ill?

Firstly, thanks Lucy, you had always be the 1st one to give respond to my threadsNot to forget Alex, thank you very much

Ops,I got my readings wrong(in fact I just accidentally copy paste it )
I'm using Tetra Test 6 in 1 (paper type).

Nitrate:25 mg/l
Nitrite:3 mg/l
GH:4
KH:4
Ph:below 7
chlorine:0

His gill had been that way for about 1 month now after the accident.

My other fish are (5 neon tetra,3 platy n 3 corys) fine I guess as there are no changes in behaviour, swim and eat normally.
What should I do?I'm thinking going to the pet shop but I don't really know what I need to buy.

I forgot to say that his one eye does look bulgy like it had translucent film over the lens of the eye. Could this be it?

NH3 reading? NO2 at 3ppm is considered toxic, Why so high if tank is well established tank.
Tank water is very mulky/cloudy. Wondering you may have disturb the tank too much.

Since you are leaving in 2 days, how about doing 25% water change (do not vacuum gravel, do not touch filter media)once in the morning and one in afternoon and cut down on food. Dont know how long is your vacation, ,more concerned with aphyxiation/acidosis due to high NO2 exposure than starvation.

How about adding Aq Salt, 1 tea/5G and compensate correct amount w/ each water change (if done 10 G pwc, then add 2 tea). Salt will help you detox NO2 since Na+ will bind with NO2- thus less toxic ionic form.
I do not know how you set up Q/T but add some gravel from main tank in hoping that Q/T stablize faster.

As far as medication, very difficult choice choosing b/n detoxicing nitrite poisoning or treat for eye problem.
If was mine. would detox first by using Methylene Blue. Known to work and have used them when receiving new fish with great results. Follow the direction on bottle carefully. Cant remember the dosage and duration..
Lets hope main tank stablizes before you leave.
