I once found this tropical fish Disease Chart and I think it is very comprehensive. I believe it contains all freshwater diseases described. It has the information on the symptoms, diagnosis, and treatment. I think it is a great reference source for those suspecting they have sick fish. So I am posting it for those who may need it.

Just in case anyone asks,
OTC medication in the "Treatment" column means "Over The Counter". It is basically whatever the medication your fish store has for the disease you know your fish has. Of course, just because you have this chart here, does not mean that you should not ask about diseases on the forum to make sure that your diagnosis is correct. The chart I gave you is just to help you out. I think it's very useful.
P.S.1 Here is another useful website with diseases. It also gives the names of
specific medications to treat various fish diseases. Here it is:

P.S. 2 And here is yet another great website which Atm told me about
:
http://www.2cah.com/pandora/Disease.html#Cloudyeye It contains the
PICTURES of all the diseases, so I hope it will help many of you who are not sure what disease your fish has. (
Note: For some reason, this link will not work sometimes. If it doesn't work for you at a certain time, try again at a different time.)
P.S. 3 And one more informative website - suggested by JMatt1983 - that also includes
