Does the spot look fuzzy (Fungus) and like it's raised off her body or does it look more like a shallow bit of her scales have come off (Probably an injury)?
It does't look like ick. It could be fungus as COBettaCouple says. I think something definitely is the matter as she seems to be swimming in a lathargic way or in one place. I she?
If the ammonia & nitrites are 0 and the nitrates 10 or less then you are maybe looking at internal parasites or internal bacteria but those both will show symptoms in the fish.
What are you using to treat your tap water and did the fish show any symptoms?
I'm not sure on the Molly, other than fungus (Columnaris) which can sometimes be slick and not fuzzy.

I use rain water from the tank. Not tap water.
So far my water quality is good. Been testing and changing water regularly.
But this week I have lost 2 fishes already - 1 balloon ram and 1 blue gourami.
At least I knew what happened to the Balloon Ram.
But I don't know what happened to the Gourami.
As for Molly, I manged to isolate her and gave her Methylene Blue.
What is your regular water changing schedule? I do about 40% weekly.
Meth. Blue is ok to add, just be aware that it can stain almost anything so you'd not want to spill or drip any of it and a little bit of it goes a long way, but the dosage instructions should be on the bottle.
That's definitely not too often for water changes. I would still recommend Prime since some ammonia did show up, it will deal with heavy metals that you can't test for and that can be present in rainwater and it will provide slime coating enhancement for your fish.
