Angel fish bloated?

Should I be concerned about one of my black angels? It's looking bloated for about 2 days, not sure if it's disease or full of eggs? He's not eating as well also, but will eat blood worms. I did lose a few other angels last month, not at the same time but all within a week or so. The other angel that died stopped eating all together. Should I wait to see if it's holding eggs? if so how many days? or should I setup a 10 gallon and treat it with antibotics? should I treat the ones that aren't showing any bloating also?

what test kit are you using and what are your nitrites?are the white spots on the fish or part of the camera's picture?

whenever a fish doesnt look like its doing good, a nice 30-40 % water change for a few days, with some prime, amquel+ or ammo lock wont hurt ...adding some stress coat+ also will help

No the spots are on the fish, I know it's not ICH, it's the scales on the fish. I've had many angel fish over the years but never had a problem like this one. I heard about the angelfish disease but never experience it. I want to treat the tank which is 46gallons but not sure what to treat it with? I just did 15 gallon water change the other day and I always use Aqua plus for water treatment.
I just found one of clown loaches dead too.
I had this tank empty for over a year due to health reasons and just started it up a few months ago, now it looks like all the fish I've put in are dying, except for the neons which look fine as of now.
